In the final session of The Ohio Council’s 2022 conference series, Dr. Patrick Runnels will share information about relationship-based case management and explain how it is a key tool for patient engagement and improving patient outcomes. Effective case management for patients with chronic conditions leads to better patient health and satisfaction, a reduction in avoidable re-admissions, and improves overall patient outcomes.
