DESCRIPTION

In the final session of The Ohio Council’s 2022 conference series, Dr. Patrick Runnels will share information about relationship-based case management and explain how it is a key tool for patient engagement and improving patient outcomes. Effective case management for patients with chronic conditions leads to better patient health and satisfaction, a reduction in avoidable re-admissions, and improves overall patient outcomes. Participants will learn strategies to develop trust with patients and better understand their complex medical needs resulting in improved management of health conditions.  Attendees will leave this session with a better understanding of how relationship-based case management differs from traditional case management, strategies to implement this model, and how it can be used to improve individual and health system outcomes. Learn more about Patrick Runnels, MD, MBA.

LEARNING OBJECTIVES

  • Understand the barriers that patients with complex medical, social and behavioral health issues face, particularly the barriers of trust, self-esteem and complexity
  • Consider the difference between transactional, reactive care and relational, proactive care
  • Discuss how health systems' traditional approach to complex patients has not worked
  • Describe longitudinal, relationship-based case management, contrast it with traditional case management, and discuss how it impacts patients
