Provides nursing and med-somatic/pharmacological management, as well as behavioral health services to patients/consumers, including in those in the transitional housing programs. Responsible for working within the scope of their license in accordance with policies and procedures, and current federal, state, and local standards, guidelines, and regulations. Required to have Active NPI number, Ohio Medicaid Billing Number, Ohio Nursing Board License 

Essential duties and responsibilities:

  • Addresses the behavioral and physical health needs of patients receiving treatment for substance use disorders and other co-occurring disorders.
  • Performs health care assessments and screenings for the purpose of recording patient medical histories and physical conditions.
  • Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration.
  • Reviews medication effectiveness of patients and monitors and administers injections and other medications, as needed.
  • Assists patients with organizing treatment resources and provides patients with health and wellness, as well as pharmaceutical training and education to promote the overall physical and psychological wellness of the patient.
  • Responsible for maintaining the schedule of patients being seen by the Physician, Nurse Practitioner, Physician Assistant, etc. for MAT and other medical or mental health services.
  • Responsible for medication storage, medication returns, medication destruction, and calling pharmacy to resolve medication issues.
  • Orders medical supplies, tracks inventory of medical supplies, and maintains/organizes supply room for medical supplies.
  • Monitors and track lab orders, outside medical consults, as well as tracks medical transfers of patients.
  • Collaborates with other treatment professionals to promote optimal patient health, benefits and supports.
  • Empowers patients to increase pro-social decision-making capacity and overall independence.
  • Reports on any observed changes in the patient’s mood or behavior, healthcare-related needs, or mental health-related needs.
  • Notifies the Clinical and Medical Teams of any changes in the patient’s circumstances and maintains all records as instructed by the company’s policies and procedures.
  • Reports any incidents, complaints or abuse regarding a patient, staff member and/or vulnerable adult.
  • Attends and participates in scheduled or required staff meetings, training, peer reviews, workshops and supervision sessions while contributing to the overall effectiveness of the team environment.
  • Completes required documentation no later than 48 hours after the service being provided.
  • Adheres to the company’s personnel policies and procedures, JCAHO, OMHAS, Medicaid and other insurance standards, and fulfills documentation and reporting requirements.
  • Informs supervisor when ill or unable to attend to job duties.
  • Identifies program needs and makes suggestions for improvements to the Physician, Nurse Practitioner, Physician Assistant for which you are working under, as needed.
  • Complies with all company policies and procedures, including but not limited to Compliance, HIPPA and Professional Code of Ethics.
  • Maintains at least their current or the minimum required Ohio licensure status as a qualified service provider.
  • All CEU certificates obtained during employment with CRN Healthcare, Inc. must be copied for entry into the personnel file.
