We are seeking passionate social workers to become part of an integrated services team within the University Hospitals Portage Medical Center emergency department. The Behavioral Health Navigator will assist in the identification of behavioral health needs in presenting patients through brief screening, diagnostic assessment, crisis support and intervention. This position contributes to disposition planning and level of care determination for patients and provides linkage and referrals to the appropriate resources. Duties include:

  • Use triage, engagement, assessment and trauma-informed behavioral health interventions to foster crisis intervention
  • Work collaboratively with a multidisciplinary team to connect individuals with both inpatient and outpatient healthcare and behavioral health care services
  • Maintain a collaborative relationship within community referral sources, hospitals and community stakeholders

Qualifications:

  • Must have Ohio licensure as LSW
  • Must be able to work flexible shifts with weekend and holiday rotation
  • Excellent driving record

Preference given to the following candidates:

  • Past or current work experience in an emergency room, community behavioral health or crisis intervention setting
  • Past or current experience in motivational interviewing, crisis intervention, and trauma informed care
  • LISW preferred

This crucial position and the expansion of our crisis continuum is available thanks to a nearly $4 million SAMHSA grant that Coleman received to implement Certified Community Behavioral Health Clinics (CCBHCs) in our service areas of greatest needs.
