Residential Case Manager - Full Time QUALIFICATIONS:

  1. High School Diploma; Undergraduate degree in human service area preferred.
  2. Minimum CDCA licensure required.
  3. Knowledgeable about Substance Use Disorders and community resources.
  4. Experience in residential treatment setting preferred.
  5. Computer literacy required.
  6. Excellent communication and organizational skills.
  7. Must be able to work evenings/weekends and off site as scheduled.
  8. Reliable transportation.
  9. Valid Ohio driver’s license required.

AREA OF RESPONSIBILITY: Provides supportive activities to enhance a residents’ long term recovery from substance use disorders.

  1. Assists residents in gaining access to needed health and dental services, financial assistance, housing, employment, education, social services, and other services and supports developed through the individualized treatment planning process.
  2. Interacts with residents in a supportive manner, encouraging discussion and activities focused on recovery.
  3. Schedule and transport to medical, mental health appointments or other transportation as necessary.
  4. Assists with screening and intakes of potential residents as needed.
  5. Assists with assessments, treatment planning, case management, individual, and group counseling as assigned/documented by supervisor, and depending on licensure.
  6. Works with treatment team to ensure that each client’s transition plan is consistent with their individualized treatment plan.
  7. Planning, linkage and coordination of referrals to various support services; monitor residents progress in services; provide client advocacy.
  8. Acts as liaison with referral sources.
  9. Manage normal residential treatment functioning and emergency situations independently.
  10. Maintains professional standards of ethics. Thorough knowledge and implementation of applicable laws and governmental regulations relating to client confidentiality, client rights and grievance policies and procedures.
  11. Completes client charting and other required documentation in a timely, professional manner.
  12. Accurately track outcomes and complete residential treatment follow-up reports as part of agency Quality Improvement Plan.
  13. Participates in all agency and residential treatment staff meetings as required, and in-services as scheduled.
  14. Maintains certification and/or licensure, as applicable

$28,000 - $42,000 annually
