Adult Mental Health Case Manager (Jefferson County)

Coleman Health Services – Steubenville, OH

Seeking an individual to provide intensive, case management services to individuals with severe and persistent mental health issues in Jefferson County. 

The Case Manager will assist with accessing community-based resources, housing, employment, making appropriate referrals and linking the individual with ongoing mental health services.

Must have the following: a NPI number; and an Ohio Medicaid number and BA/BS in mental health or related field or three years experience in mental health or related field. Must have and maintain excellent driving record. Experience in case management preferred.
