We have an excellent opportunity for a full-time Mental Health Counselor to join our team in our Youth Recovery Program. This position provides direct mental health treatment services to clients in an outpatient setting and the activities in the Youth Recovery Program. 

KEY RESPONSIBILITIES

  • Provides individual, group, and family counseling under clinical supervision that meets productivity and quality requirements set by the Agency and that is within the limits of services permitted by license and scope of practice.
  • Develops and implements individual and group activities designed to resolve and stabilize decompensating psychiatric conditions. Provides educational, rehabilitative, and therapeutic activities designed to facilitate a return to, or continuation of, family, education, and/or community activities.
  • Provides diagnostic assessments under clinical supervision that meet productivity and quality requirements set by the Agency.
  • Meets or exceeds standards pertaining to assessments and documentation.
  • Provides only those services that have been authorized by Utilization Management and provides services within the authorized frequency and duration of services.
  • Requests additional services when initial authorization has been met.
  • Written assessment of individual’s history, mental status, strengths, needs and resources including recommendations for treatment completed within 24 hours of the assessment.
  • Assessment and other documentation will provide sufficient reasoning for diagnosis and the type, frequency, and duration of services provided.
  • Individualized Service Plan and Discharge Criteria with specific goals and objectives that are measurable are developed with the consumer and signed at time of the initiation of services and at time of each update.
  • Assessment, Individualized Service Plan, Progress Notes and other documentation clearly identifies consumer’s input in services by having consumer’s and parent’s/guardian’s (if applicable) signatures when clinically appropriate.
  • Completes progress notes at the end of each session and turn in within 24 hours of completion.
  • Notifies consumer and referral source (when applicable) prior to completion of the discharge summary and document this notification on discharge within 30 days of last contact.
  • Develops and maintains quality, accurate clinical charts for all clients served.
  • Develops, coordinates and implements daily activities for YRP groups in accordance with the YRP curriculum.
  • Is continually available by having no more than two unscheduled incidences of absences and/or tardiness per three-month evaluation period.
  • Develops and maintains positive collaboration with other social service agencies serving clients.
  • Provides consultation if needed.
  • Has no unexcused absences from assigned clinical and administrative committee meetings and training.
  • Maintains license or registration appropriate to profession and appropriate to job requirement, including appropriate amount of continuing education at all times of employment.
  • Maintains agency-required annual training including, but not limited to, client rights and grievances, safety training, infection control, and others as required by OhioMHAS.
  • Remains free from citations by other providers or by consumers for unethical practice and meets standard for own profession.
  • Adheres to all organizational and departmental policies, including compliance with all behavioral and ethical expectations.
  • Participates in weekly supervision with assigned supervisor; in accordance with TCN policy and procedures and the requirements of supervision as defined by individual license/registration.
  • Other clinical duties as assigned. 

MINIMUM QUALIFICATIONS

Education:

  • Bachelor Degree (higher PTO level offered with Master’s Degree) 

Experience:

  • Two or more years experience in mental health services 

Certification:

  • LSW, LPC, or RN; CPR; First Aid; De-escalation.4. 

Other:

  • Meets all standards and regulations of the Counselor and Social Worker Board of the State of Ohio.
  • Demonstrated competencies for working with severely emotionally disabled youth.
