Coleman Professional Services is seeking a social work or counseling individual to be the Director of Integrated Health which will oversee the implementation and delivery of integrated care services within key partner clinic and supervises a team of direct service providers.


Key duties include, but are not limited to the following:

Administration and Management:

  • Direct the treatment of services for assigned clients as defined by the agency standards
  • Prepare clinical performance reviews, and ensure completion of individual new employee orientation and ongoing training for direct reports
  • Provide supervision/guidance/crisis support to direct reports as required by agency policy and provide individual and group clinical supervision as required
  • Implement strategic vision for program growth and cultivate new partnerships/contracts
  • Develop and maintain an understanding of evidence-based models in integrated care and emergency services
  • Monitor/maintain compliance and oversee process improvement for contract services
  • Maintain community relationships with key stakeholders and act as liaison for contract facilitation
  • Perform data collection and presentation as required by contract agencies
  • Provide consultation/educational information with other community agencies/organizations
  • Follow, support, and aid in the development of personnel policies and standard operating procedures Resource Development
  • Attend intra- and inter-agency organizational meetings, and serve on committees as assigned
  • Develop and maintain collaborative relationships with community referral sources, hospitals, and community stakeholders Direct Care
  • Complete mental health crisis intervention and pre-hospitalization screening as needed
  • Document clinical work through progress notes and other forms as appropriate


Qualified Candidates will have:

  • Ohio LISW or LPCC;
  • At least 3 years experience in indirect service;
  • Experience with SMD population;
  • A NPI number, Medicaid number, and CAQH account.
  • In addition, must be able to communicate effectively with client population, multi-disciplinary staff, and partner agencies;
  • Familiar with community system and agencies;
  • Prefer someone with business development experience.
